SxM is a full spectrum clinic specializing in anesthesia, pain medicine, palliative care, symptom management and hospice.  Patients can be evaluated in the clinic, at the hospital, and under special circumstance in the home.  We are available to discuss pain or symptoms concerns you may have about your patient.  To initiate an appointment, complete the referral form.  Supporting documents and forms may be faxed 1888.SXMMED.0 (888.796.6330).  Please contact us if you have any questions phone: 1888.SXMMED.1 (888.796.6331).


We can provide consulting services for of the following:

  1.     Pain Medicine, including acute & chronic pain, and challenging post-op

      pain control

  1.     Cancer pain and symptom management

  2.     Palliative care for chronic disease

  3.     Hospice and end stage disease pain and symptoms

  4.     Interventional pain procedures and injections

  5.     Anesthesia services, including pre-op evaluations for surgical readiness

  6.     Medication Management


To get started, the clinic has some requirements:

  1.     A completed consultation or procedure  referral  form

  2.     Current H&P and supporting documentation (i.e. MRI, x-rays, labs, meds

  3.     Insurance pre-authorization for a consultation or procedure

  4.     The patient must be under the care of a PCP at all times, and once the        

      procedure is completed, all medical care will continue with patient’s PCP
